COVINGTON, Ga. — Kids are bound to get some scrapes and bruises on the playground, but a playground incident at an elementary school in Georgia ended up killing a young girl.

Andrea Tyrah Debruhl was playing on the monkey bars at school and slipped, WSB-TV reported.

Teachers found her hanging from the bars, unconscious, and an autopsy report showed she died of asphyxiation.

Andrea was only 5 years old and in kindergarten.

It was ruled an accidental death, but investigators are trying to figure out how she ended up in the position that killed her.

There were 10 teachers and other supervisors on the playground at the time, according to the school district.

“You think playgrounds are safe for kids,” a parent told WSB-TV. “You wouldn’t think monkey bars would be a cause.”
